No Provider-Patient Relationship

Using this Site, submitting a referral form, or checking insurance acceptance does not create a provider-patient relationship. A provider-patient relationship is established only when you are formally accepted as a patient and have completed our intake process.

Appointment Scheduling

Appointment scheduling on this Site is powered by IntakeQ, a third-party platform. By using the scheduling tool, you also agree to IntakeQ's terms of service. We are not responsible for any issues arising from IntakeQ's platform or availability.

Referral Submissions

Referrals submitted through this Site are not guaranteed to result in a patient appointment. We will make reasonable efforts to contact referred patients within 48 hours, but submission of a referral does not guarantee acceptance into our practice. For urgent or emergent psychiatric needs, referring providers should direct patients to the nearest emergency department or call 988.

Insurance Information

Insurance acceptance information displayed on this Site is provided for general guidance and may not reflect real-time changes in our network participation. We make reasonable efforts to keep this information current, but we do not guarantee its accuracy. Always verify coverage directly with your insurance plan before scheduling an appointment.
